2010 Summer Camp Parent Guide

Health Form  (Required for all campers)

In addition to the Health Form above, the Physical Exam Form is required if your camper is a:

• Resident Camper attending Chestnut Ridge for the first time
• Resident Camper whose doctor's signature on a previous year's form is for a physical exam done more than 24 months prior to camp
• Day Camper participating in an off-site program (e.g. the Rock & Raft trip)
